The surgeons of Mid Atlantic Surgical Group are available for new patient consultations by request.
If your doctor's office wants to refer you to our office your doctor may set up a consultation appointment for you. Or if you do not have a referring doctor or would like to make your own appointment with us directly you may contact our office directly to request a consultation.
Our staff will assist you with any insurance needs.
New Patient Paperwork
If you are coming to see us for the first time, please click the appropriate physician name to print out forms that you can fill out in advance in order to speed up the check-in process.
If you've had any imaging studies and/or other tests related to your consultation we will need those results for your consultation. Please be sure to bring any of those testing reports and/or discs with you to your appointment.
